    Call  (714) 397-0467 or email  Therapractics@sbcglobal.net  to set an appointment.  We try to accomodate you if at all possible.  Visiting our Therapractics.com website "NewPatient Center" category and filling out the necessary history forms online and either faxing them to (714) 957-1347 or emailing them back to me at Therapractics@sbcglobal.net prior to the appointment time is essential before the evaluation and treatment can occur. 

    Your health or current condition along with a brief history will allow the Doctor a better understanding of what condition he will be treating in general.  This will allow the Doctor to pull the appropriate forms required.  Your cooperation in this matter will be greatly appreciated and serve to help prepare the Doctor for his initial visit with you.
